Complete a physical form with your doctor. Students only needs one physical, this is good 6-8th grade. Physical Form 3. Form B-History Form needs to be completed every year. No doctor signature required. Form B-History Form 4. Athletic Release Form completed for each sport in order to pick up your student from games.
